The Hawai‘i Department of Health partners with community-based organizations to provide case management and supportive services to people living with HIV as well as their spouses, partners, and families. To access HIV supportive services, such services funded in Hawai‘i through the Ryan White HIV/AIDS Program, an individual must enroll in a case management program. Case managers provide:

Medical Case Management

Medical Case Management to help individuals living with HIV to access primary medical care and removing barriers to remaining in treatment. Activities include assistance and support with:

• Applying for entitlement program benefits such as Medicare, Medicaid, Veteran’s Administration
• Accessing HIV medical management services: HSPAMM, HDAP and H-COBRA
• Accessing health-related services such as medical care, medical nutrition therapy , dental care, mental health services, substance use services, and home health services
• Accessing support for transportation, food & housing

Specialized Case Management

• Individuals who have substance use and/or mental health issues and individuals who are homeless
• culturally sensitive and culturally appropriate for Native Hawaiians
• Women, Children & Families

Social Services Case Management

• Assessment, counseling, referral, advocacy and follow up on a range of social service issues

Client Support Services

• Food bank, housing assistance, and medical transportation
