The Harm Reduction Services Branch has updated their case report form for sexually transmitted infections (STIs). Click here to download the new form.

What’s included in this update:
– Treatment options that reflect the CDC’s new 2020 Gonorrhea Treatment Guidelines
– Option to report syphilis infections by stage (primary, secondary, early latent, unknown) and clinical manifestations (neurologic, ocular, etc.)
– Form-fillable fields allow for faster entry by the healthcare provider
– Additional page containing helpful information on CDC recommended antimicrobials, sex partner management, local reporting laws, gonorrhea treatment failures, and HDOH’s STI Information Line
