Report a Pesticide Poisoning (for Healthcare Providers)
Per Hawaiʻi Law via HAR 11-5, physicians and laboratories must report known or suspected pesticide-related illness and injury to the Department of Health within 24 hours.
PHYSICIANS
- Preferred Method: Call the Hawaiʻi Poison Center 1 (800) 222-1222.
- The Department of Health has an agreement with the Hawaiʻi Poison Center to inform the health department on the provider’s behalf.
- Alternate Method: Report by phone to the Hawaiʻi Department of Health (808) 586-4249 on Oʻahu, or 1-800-468-4644 ext 64249 from the Neighbor Islands.
- Alternate Method: Fill out the Pesticide Exposure Assessment form or the fillable version of the form and mail to:
