The Department of Health, Developmental Disabilities Division’s (DDD) supports each participant’s trajectory toward an inclusive, quality life in the community. Through partnerships with participants, their families and their communities, achievement of possibilities throughout a person’s lifespan can be supported in a variety of ways. DDD helps people to think about how to work with others, and to use their personal strengths and assets to realize their vision for a good life. Through person-centered planning, we work with people to identify what they need to achieve their goals including in the areas of their relationships, use of technology and community resources, and use of eligibility-specific supports such as services through the Medicaid 1915(c) Home and Community-Based Services (HCBS) Waiver for individuals with intellectual and developmental disabilities (I/DD). Support to families of participants is a very important aspect of achieving each individual’s vision of a good life in the community. Families will be supported in ways that maximize their capacity, strengths, and unique abilities to best support the individual to achieve their goal. Together, we work to facilitate opportunities for the achievement of self-determination and inclusion of individuals and families in all facets of community life.
