Medical Providers – Getting Started

The DOH Medical Cannabis Registry Program is a Registration Program. This means:

  • Medical Providers must certify in writing that in their professional opinion the qualifying patient has a debilitating medical condition and the potential benefits of the medical use of cannabis would likely outweigh the health risks.
  • Qualifying patients (and their primary caregivers, if applicable) shall register with the DOH, and DOH shall issue the qualifying patient a registration certificate (the 329 Card).
  • DOH must provide access to registry information to Law Enforcement for official Law Enforcement purposes.


DOH recommends
that a potential qualifying patient discuss with the medical provider that is currently treating them for that condition whether or not medical cannabis is the right treatment option for their condition.

  • Before certifying patients, medical providers must first register on the Medical Cannabis Registry website.
  • All certifying Physicians must have a Valid Hawaii Medical License Number & a Valid Hawaii Controlled Substance License Number.
  • All certifying APRNs must have a Valid RN License Number & a Valid Hawaii Controlled Substance License Number.

Directions For First-Time Medical Providers:

1. Physicians and APRNs should review HRS, chapter 329 part IX, and HAR 11-160 before registering with the Hawai’i Department of Health.

2. Go to MyPVL https://mypvl.dcca.hawaii.gov/ login or sign up for an account. You must link your MD/DOS/APRN license to your MyPVL account, if not previously linked, before creating your first online application. Once correctly entered, your linked license will display on your MyPVL dashboard.

3. Go to Medmj.ehawaii.gov. If it is your first time to log in, click the “First Time Physician/APRN Log In” (located on top right of the screen). Enter your information in all fields. The email password will be the same used for MyPVL.

4. Complete the Electronic Signature Agreement and mail the completed form to the Department of Health (address provided on form).

5. Mail the completed form to:

The Registration Process allows the Medical Provider to:

  1. Review Patient Applications
    For Patient-initiated Applications, the patient will create a medmj.ehawaii.gov account to submit the application and pay the DOH application fee. Once submitted, the app will be sent to their certifying provider’s pending queue. (THIS IS THE RECOMMENDED WAY TO SUBMIT A PATIENT APPLICATION.)   2. Return a patient’s application to a patient’s medmj.ehawaii.gov account if a correction is needed.
  3. Certify their patient’s medical condition and send the application to DOH for review & approval.
  4. Upload documents on behalf of a patient (if they choose to).
    IF a patient does not have technical access to create a medmj.ehawaii.gov account, a Physician-initiated Application can be created and submitted by the Medical Provider on the patient’s behalf.
