County of Kauai Disability Parking Permit Information

Due to the seriousness of COVID-19, all disability parking permit applications for the  County of Kauai may be submitted:

  1. In person at the Kauai County Motor Vehicle and Licensing office during normal business hours with an appointment (see below); OR
  2. By mail (see below for mailing instructions).

Please read the following options below carefully. Sending your application to the wrong agency will delay your parking permit request. Disregard instructions listed on application forms until further notice.

In person: 

Make an appointment by clicking on the Kauai DMV Website

Select “Drivers License” and then “Other Appointments (Permits, DCAB, Taxi)”

 

By Mail:

Long Term (Blue) Placards
Applying for a first time long term (blue) placard: 

  • Complete form PA-1 (physician/APRN to fill out Side 2)
  • Make a copy of valid identification card
  • Mail both items to:
    • Department of Finance – DL Office
      Attn: Tami

Renewing a long term (blue) placard (it is expired or will expire):

  • Complete form PA-2 (physician/APRN to fill out Side 2)
  • Make a copy of valid identification card
  • Mail both items to:
    • DCAB

Replacing a long term (blue) placard (lost or stolen):

  • Complete Side 1 of form PA-1 (physician certification is not needed if placard is still valid)
  • Make a copy of valid identification card
  • Send a $12.00 check payable to: Director of Finance
  • Mail all three items to:
    • Department of Finance – DL Office

Replacing a long term (blue) placard (mutilated):

  • Complete Side 1 of form PA-1 (physician certification is not needed)
  • Make a copy of valid identification card
  • Send mutilated placard
  • Mail all three items to:

Temporary (Red) Placards
Applying for a first time temporary (red) placard:
