County of Hawaii (Hawaii Island) Disability Parking Permit Information

Due to the seriousness of COVID-19, disability parking permit applications for first time, temporary, and replacement placards for Hawaii County may be submitted:

In person at any Hawaii County Office of Aging office during normal business hours. Applications for first time, temporary, and replacement placards can also be mailed to the County offices listed below. All applicants must:

  • Complete form PA-1 (physician/APRN to fill out Side 2)
  • Make a copy of valid identification card
  • send a $12.00 check payable to: County Director of Finance (REPLACEMENT AND TEMPORARY PLACARDS ONLY)
  • Mail or bring in both items to the offices listed below:
    • Hilo: 1055 Kino’ole Street, Hilo, HI 96720 7:45 a.m. – 4:30 p.m.
    • Kailua-Kona: West Hawaii Civic Center 74-5044 Ane Keohokalole Hwy., Bldg. B, Kailua-Kona, HI 96740

Renewals of Long Term (Blue) Placards must be submitted by mail.

Renewing a long term (blue) placard (it is expired or will expire):

  • Complete form PA-2 (physician/APRN to fill out Side 2)
  • Make a copy of valid identification card
  • Mail both items to:
