Clinical Laboratory Personnel Licensing

Welcome to the Hawaii Department of Health State Lab License System. Here you can apply or renew your laboratory license online, download licensing instructions and requirements as well as links to sites where you can take your certification exams.

Due to current staffing shortages, processing of clinical laboratory personnel license renewals will be delayed until further notice. Currently, the Hawaii Department of Health (HDOH) is unable to accept credit card payments for renewals, though we are actively working to have this option available soon.

To help us serve you more efficiently, please avoid sending email inquiries about license renewals, as we are unable to respond to individual messages at this time.
