MEDICAL PROVIDERS – Fees

Pursuant to Act 241, Session Laws of Hawaiʻi (SLH) 2025, the fee for medical cannabis certification—which includes both the evaluation and the issuance of certification—is capped at:

  • $105 for 1-year certifications
  • $210 for 2-year certifications

Any fees assessed to issue a written certification shall not exceed an amount equal to three times the Department of Health’s (DOH) registration fee. The current DOH registration fee is $35 for 1-year certifications and $70 for 2-year certifications.

Guidance on Medical Cannabis Certification Fee Cap

The following activities are considered part of the certification process and must be included in the capped fee:

  • All aspects of the evaluation for the medical use of cannabis, including explaining the potential risks and benefits of medical cannabis use.
  • Completion of the written certification using OMCCR-approved forms.
  • Maintenance of medical records related to the certification.
  • Providers must inform patients of the certification fee and any additional fees prior to conducting the evaluation.

Other Medical Services

  • If additional medical services unrelated to medical cannabis certification are offered, providers may charge a separate fee.
  • Any additional medical services must be disclosed to the patient prior to providing the services.
  • Any additional fee must be disclosed to the patient prior to services being provided.
