In cooperation with the Hawaiʻi Breast and Cervical Cancer Control Program (HBCCCP), the Well-Integrated Screening and Evaluation for Women Across the Nation (WISEWOMAN) program offers free heart health screening and evaluation services to decrease their risk for heart disease and stroke. This is an extension of services to women ages 35-64 enrolled in HBCCCP to increase access to integrated care in one location.

Program Enrollment

After a woman is enrolled into WISEWOMAN through HBCCCP, a provider will evaluate their risk of cardiovascular disease. Following the evaluation, women are referred to the appropriate lifestyle support services to help control, delay, and prevent heart disease and stroke and/or are also provided with community-based resources to help address social determinants of health. These programs are accompanied by a health coach to help women make healthy behavior changes by promoting healthy eating, physical activity, support for tobacco cessation, and medication adherence assistance.

Services Provided

  1. Screenings for heart disease and stroke risk factors
  2. Counseling to reduce risk for heart disease and stroke
  3. Patient referrals to healthy lifestyle programs, community based resources, and other healthy behavior support services

Risk factors for Heart Disease and Stroke

Addressing risk factors such as high blood pressure, elevated cholesterol, physical activity and nutrition, diabetes, and smoking greatly reduces the risk of cardiovascular disease-related illness and death for women.

From 2019 – 2023, heart disease was the leading cause of death in Hawaiʻi. Other risk factors include:

  • Eating a diet high in saturated fats, trans fat, and cholesterol
  • Too much alcohol
  • Genetics and family history
