Immunization Requirements Summary – additional information on the immunization requirements, including number of doses required and minimum spacing between doses. Updated May 29, 2020:

  1. Cover Page: Added last bullet with contact information for Hawaii Department of Health, Immunization Branch
  2. Table 1: Clarified that if children did not receive Hib or PCV at the routinely recommended ages, should refer to Tables 5A and 5B
  3. Table 4: In Notes section for Diphtheria, Tetanus, Pertussis, added: “A 5th dose is not needed if 4th dose is given at age >4 years and at least 6 months after DTaP 3.” Changed minimum age for Tdap from 11 years to 10 years.
  4. Table 5A: Added information regarding children who did not receive Hib according to the “Late Start Schedule,” along with links to CDC’s Catch-Up Guidance.
  5. Table 5B: Added information regarding children who did not receive PCV according to the “Late Start Schedule,” along with a link to CDC’s Catch-Up Guidance.
