Reporting an Illness: For Healthcare Providers

As mandated by Hawai‘i Administrative Rule (HAR) , Title 11, Department of Health, Chapter 156, Communicable Diseases and its exhibits, certain infectious diseases are reportable in Hawaii. Health care providers caring for a person with a diagnosis, or provisional diagnosis in the absence of definitive tests results for confirmation, shall notify the State of Hawaii department of Health. Please refer to HAR Title 11, Chapter 156 Exhibit A.

Specimens to be submitted for testing for diseases reportable to the State of Hawaii must be pre-approved by the Department of Health Disease Investigation Branch prior to submission to the State Laboratories Division.  Contact Disease Investigation Branch at (808) 586-4586 for further information.
