Immunization Requirements Summary – additional information on the immunization requirements, including number of doses required and minimum spacing between doses.

Updated May 29, 2020:

1. Cover Page: Added last bullet with contact information for Hawaii Department of Health, Immunization Branch

2. Table 1: Clarified that if children did not receive Hib or PCV at the routinely recommended ages, should refer to Tables 5A and 5B

3. Table 4: In Notes section for Diphtheria, Tetanus, Pertussis, added: “A 5th dose is not needed if 4th dose is given at age >4 years and at least 6 months after DTaP 3.” Changed minimum age for Tdap from 11 years to 10 years.
