Hawaii Diabetes Stakeholders
Two years of assessment and strategic planning resulted in the formation of the HHSP Diabetes 2030 Plan. The Hawaii Diabetes Prevention and Control Program has taken the lead in convening diabetes stakeholders across the state to encourage collaboration and partnerships to reduce the burden of diabetes in the state.
Priorities for the diabetes health system have been identified and are detailed in the Hawaii Diabetes Plan. The Hawaii Diabetes Prevention and Control Program will continue to provide opportunities for diabetes stakeholders to network, share lessons learned, and get involved addressing different goals in the plan.
The Hawaii Diabetes Plan reflects the vision of diabetes stakeholders statewide to reduce the burden of diabetes in Hawaii.
