HW Tips for completing the notification

EPA provides long, complete instructions here. Below are the quick instructions on the items people usually have questions about.
Initial and subsequent notifications can now be submitted online using myRCRAid. You will need to set up a user account in the RCRAInfo Industry App (instructions are on the main notification page).
Fill out all fields. Specific item number with tips are listed below.
7. NAICS code: look up your NAICS code here
8. Site contact information: You will receive announcements from the Department of Health at the e-mail address provided in this section.
9. Legal owner/operator: Legal owner is the property owner, usually a business, and may be a business that the operator rents from. Legal operator is the business submitting the notification form, not an individual.
10A1. State Generator Category. For Hawaii, State Generator Category is always the same as federal. You can mark Y or leave this blank.
10A2. Short-term generator: Check the box for short-term generator only if your waste is produced from a one-time or very short-term event. Short-term generator numbers are only good for 90 days after issuance. If you check this box, you MUST provide an explanation in the comments section.
10A3. Treater, storer, or disposer of hazardous waste: Activities listed in this section require a permit from the Department of Health. Please call the Hazardous Waste Section at 808-586-4226 for more information.
10A4, 10A5. Receives hazardous waste from off-site/Recycler of hazardous waste: Do not check this box if the only hazardous waste you receive from off-site is universal waste. See #11B1. Do not check this box if you are the generator of the waste to be recycled. Activities listed in this section require a permit from the Department of Health. Please call the Hazardous Waste Section at 808-586-4226 for more information.
10B. Waste codes for federally-regulated hazardous waste. These codes are found in chapter 11-261.1, HAR.
10C. Waste codes for state-regulated hazardous waste. Hawaii does not require the use of State-specific waste codes on hazardous waste shipping manifests. (Leave this section blank.)
11C. Used oil activities: Activities listed in this section require a permit from the Department of Health. Please see chapter 11-279.1, HAR and the used oil permit application (PDF); call the Hazardous Waste Section at 808-586-4226 for more information.
12. Eligible academic entities with laboratories: Hawaii has not adopted the academic laboratories rule. This section is not applicable.
13. Episodic generation: If your site is normally a VSQG or SQG and generates a larger amount of hazardous waste for a short period of time (less than 60 days), you may be able to retain your regular generator status by notifying as an episodic generator. Notification must be submitted at least 30 days before a planned episodic event and you must complete page 9 of the notification form. To see if you qualify, please read 40 CFR part 262 subpart L, as incorporated and amended in chapter 11-262.1, Hawaii Administrative Rules, and call the Hazardous Waste Section at 808-586-4226 if you have questions.
14. LQG consolidation of VSQG hazardous waste: LQGs can now consolidate hazardous waste from VSQG sites within the state that are owned by the same company. Notification must be submitted at least 30 days before you consolidate waste and you must complete page 10 of the notification form. To see if you qualify, please read 40 CFR §§262.17(f) and 262.14(a)(5)(viii), as incorporated and amended in chapter 11-262.1, Hawaii Administrative Rules, and call the Hazardous Waste Section at 808-586-4226 if you have questions.
16. Hazardous secondary material (HSM) activity: You must complete page 8 of the notification.
19. The paper form must be submitted with an original ink signature in this section.
