The goal of the Chronic Disease Prevention & Health Promotion (CDPHP) Division is to promote wellness, alleviate barriers to access services and improve the quality of life for all of Kauai’s communities through the prevention, detection and management of chronic diseases. CDPHP programs include:

    Chronic Disease Prevention

    The Department of Health’s Breast and Cervical Cancer Control Program (BCCCP) offers Breast and cervical cancer screening services to individuals who are 40 to 64 years old and are currently uninsured or under-insured. For more information visit the BCCCP webpage.

    Prediabetes means your blood sugar is higher than normal. 9 out of 10 people with prediabetes do not know they have it. The sooner you know you have prediabetes, the sooner you can take action to reverse it and prevent type 2 diabetes.

  • Take a Prediabetes Risk Test or download a copy (available in 12+ languages)
  • Find a Diabetes Prevention Program

    Chronic Disease Management

  • In Hawaii, 1 in 3 adults have high blood pressure and some don’t even know it. Having high blood pressure increases your risk of having a heart attack, stroke, heart failure, and other complications KDHO is helping community members manage their Blood Pressure through regular self-monitoring and other heart-healthy activities.     Bilingual Health Services (BHS)

    The CDPHP Kauai Team provides health education and assistance with language and cultural interpretation for Limited English Proficiency (LEP) or English as a second language (ESL) community members. Current Bilingual services include Olelo Hawaiian, Tagalog, Ilokano, Marshallese, and Spanish.

    The CDPHP team facilitates access to services through:

  • Community outreach, home visits, family/individual counseling
  • Navigating for screening services, follow-up, and referrals
  • Advocating for representation on agencies and with service providers
  • Supporting access to public health-related services
  • Who can receive no-cost Bilingual Health Services? The CDPHP team assists immigrants, migrants, Native Hawaiians, and people from the Compacts of Free Association (COFA) communities.

    How to request Bilingual Health Services? Please contact us at the Kauai District Health Office at (808) 241-3421 to coordinate services, no referral form is needed.

    For interpretation services, please reach out to private providers or the Office of Language Access for more information.
