ELIGIBILITY AND REFERRAL
Who are eligible for services?
Eligible children and youth must:
- Be under age 21 years
- Reside in Hawaii
- Have a chronic health condition that has lasted (or may last) at least one year
- Needs specialized medical care
To receive financial assistance families must meet family income limits
