Children and Youth with Special Health Needs Section promotes family-centered, community based comprehensive, coordinated care for children and youth with special health care needs from 0-20 years.
Children with Special Health Needs Program
Children with Special Health Needs Program (CSHNP) provides care coordination, social work, nutrition, and other services for children with special health care needs age 0-21 years with chronic medical conditions. It serves children who have or may have long-term or chronic health conditions that require specialized medical care, and their families.

- Information and referral
- Service coordination related to medical specialty care
- Nutrition services
- Hearing and vision special services
- Transition to Adult Health Care
- Neurology, cardiac, and nutrition clinics on Neighbor Islands
- Limited financial assistance for specialty care related to the child’s condition (if family meets income limit)
- Medical specialist office visits
- Laboratory tests
- X-rays, EKG, EEG
- Prescription medications
- Hearing aids and related services
- Travel and lodging expenses for Neighbor Island children requiring medical services on another island
Who are eligible for services?
Eligible children and youth must:
- Be under age 21 years
- Reside in Hawaii
- Have a chronic health condition that has lasted (or may last) at least one year
- Needs specialized medical care
- Examples of eligible conditions: cerebral palsy, complex cardiac (heart) conditions, hearing loss, inborn errors of metabolism, seizure disorders, etc.
- Examples of conditions and services that are NOT eligible: ADHD, autism, cochlear implants, glasses, learning disabilities, mental health conditions, surgery, most medications, etc.
