Vital Records

We will assist with instructions to request/order certified copies of marriage, civil union, birth, and death records.  Marriage, civil union, birth and death certificates may be requested using our online system at:   https://vitrec.ehawaii.gov/vitalrecords/.

Special Note: Effective February 1, 2026, the Hawaii Department of Health will no longer maintain records of DIVORCE. (Please click HERE for more information)

 

  • Marriage License or to locate a Marriage License Agent: Please go to https://health.hawaii.gov/vitalrecords/marriage-licenses/
  • Registration of Home Births, by Appointment only: Please call 808.553.7870
  • Voluntary Establishment of Paternity or Affidavits: Please call the MDHO Vital Records office at 808.553.7870, or the DOH Vital Records, Corrections office on Oahu at 808.586.4541.
