VITAL RECORDS

Vital records is a prime customer service function of the Administration section. The staff at KDHO registers home births and issues permits for burials and cremations.

KDHO staff provides support and guidance for completing vital records (birth, death civil union, marriage, and divorce certificates, and voluntary establishment of paternity) for events that occurred in Hawaii.

Birth Certificates and Marriage Certificates

KDHO staff can now issue birth and marriage certificates if certificates are ordered online. You may order birth and marriage certificates online at: Order Vital Records. Online requests are typically processed within 14 business days. If you would like for the Kauai District Health Office to issue your Hawaii Birth or Marriage Certificate, please note the following:
– Cash or money orders are not accepted at KDHO
– All orders must be placed and paid for online at Order Vital Records
– Once your order has been completed online, forward the confirmation tracking number to: [email protected]
– KDHO will email the requestor when the order is ready for pickup at KDHO.
The Kauai District Health Office can not process in-person request forms, however copies of the request forms can be picked up at the Kauai District Health Office or are available for printing at: Request Forms. For instructions on mail-in requests, please see: DOH Vital Records.

Death Certificates

Death certificates are only issued by the Office of Health Status Monitoring, located on Oahu. For more information on death certificates, please see: DOH Death Certificates.
