SPECIALTY SUPPORT PROGRAM
Children and Youth with Special Health Needs Section promotes family-centered, community based comprehensive, coordinated care for children and youth with special health care needs from birth to under 21 years old.
Specialty Support Program
The Specialty Support Program (SSP) provides care coordination, social work, nutrition, and other services for children with special health care needs from birth to under 21 years old with chronic health conditions. It serves children who have or may have long-term or chronic health conditions that require specialized care, and their families.
SSP services may include information, referral, or support for:
- Care coordination
- Nutrition consultation
- Hearing and vision special services
- Transition to Adult Health Care
- Neighbor Island Specialty Clinics (vary by island and may include but not limited to Cardiology, Neurology, and Developmental Pediatrics)
Who is eligible for services?
Children and youth must:
- Be under age 21 years
- Reside in Hawaiʻi
- Have a chronic health condition that has lasted (or may last) at least one year
- Be under the care of a specialist (or in need of specialized services)
