SPECIALTY SUPPORT PROGRAM

Children and Youth with Special Health Needs Section promotes family-centered, community based comprehensive, coordinated care for children and youth with special health care needs from 0-21 years.

Specialty Support Program

The Specialty Support Program (SSP) provides care coordination, social work, nutrition, and other services for children with special health care needs ages 0-21 years with chronic medical conditions. It serves children who have or may have long-term or chronic health conditions that require specialized medical care, and their families.

SSP services may include information, referral, or support for:

  • Service coordination related to medical specialty care
  • Nutrition services
  • Hearing and vision special services
  • Transition to Adult Health Care
  • Neurology, cardiac, and nutrition clinics on Neighbor Islands
  • Limited financial assistance for medical care related to the child’s condition (those who are not eligible for Med-QUEST or other state or federal coverage (benefits) and uninsured)
    • Examples may include:
      • Office visits
      • Diagnostics
      • Medications
      • Hearing aids
      • Travel

Who is eligible for services?

Eligible children and youth must:

  • Be under age 21 years
  • Reside in Hawaiʻi
  • Have a chronic health condition that has lasted (or may last) at least one year
  • Needs specialized medical care
