State TB Requirements

SUMMARY OF STATE TB CLEARANCE REQUIREMENTS

In accordance with Chapter 164 of Title 11, Hawaii Administrative Rules for Tuberculosis (click to view), only the categories of people listed in the tables below are eligible for TB testing at no cost to clients through the State of Hawaii Department of Health TB testing locations (click to view the list of locations).

Once obtained, this TB clearance can be used for the purposes of foodhandling, even if change in food establishment occurs.
Exemptions:  TB Clearance not required for person engaging in foodhandling only at carnivals, fairs, and other temporary activities lasting less than 15 days.
School personnel
(includes teachers,
substitute teachers,
bus drivers, and volunteers)

 Prior to first time employment in the State of Hawaii Single TST
or
CXR if you have documentation of a previous positive TST
No expiration, once obtained, this TB clearance can be used for the purposes of school employment, even if transfer in school occurs. When transfer occurs, a copy of the original TB Clearance used for initial entry should fulfill requirement
Students
(daycare, preschool, or K-12)

CXR if you have documentation of a previous positive TST
No Expiration, once obtained, this TB clearance can be used through High School. Infants younger than 12 months of age can enter without TB Clearance Certificate, but will need to obtain a clearance between 12-14 months of age.  When a student transfers to attend another school in Hawaii, a copy of the original TB Clearance Certificate used for initial entry into school system should meet this requirement Excludes:  After-school programs, family child care, parent cooperatives, play groups, respite programs, and drop-in child care centers
Students-Post Secondary
Chapter 164 of Title 11, Hawaii Administrative Rules for Tuberculosis
Prior to first time attendance at post-secondary institution (with course work lasting 6 months or longer) in the State of Hawaii Single TST
or
CXR if you have documentation of a previous positive TST
No expiration, once obtained, this TB clearance can be used for the purposes of attendance in a post-secondary school in the State of Hawaii.
When a student transfer to attends another post-secondary school in Hawaii, a copy of the original TB Clearance Certificate initially used to enter post-secondary system will meet clearance requirement.  Also a copy of the original TB Clearance Certificate used to initially enter post-secondary school will meet clearance requirement when student re-enters post-secondary school, irregardless of the length of break between initial admission and re-entry.

NOTE:

  • For persons needing a single TST, the entire testing process takes a minimum of 2 calendar days. After your TST is administered, you must return to the same DOH TB testing location within 48-72 hours to have your TST read.
  • For persons needing a two-step TST, the entire process takes a minimum of 9 calendar days and you will receive a total of two TST’s. If your first TST is negative, a second TST is administered 1-3 weeks later.
  • For persons who have their CXR performed at the Hawaii TB Control Program on Oahu, it will take a minimum of 4 working days for your TB clearance card to be available for pickup. For example:
    – CXR on a Monday, TB clearance card available for pickup on Friday
    – CXR on a Tuesday, TB clearance card available for pickup on Monday
    – CXR on a Wednesday, TB clearance card available for pickup on Tuesday
    – CXR on a Thursday, TB clearance card available for pickup on Wednesday
    – CXR on a Friday, TB clearance card available for pickup on Thursday
  • Please bring a form of picture identification to the DOH TB testing location
  • Children under 18 years of age must be accompanied by a parent or legal guardian
  • To save time, fill out the TB Registration Form and bring it with you
