How do I submit a Records Request to the State of Hawaii, Department of Health Wastewater Branch?

Please fill out the following form (source of forms can be found at: oip.hawaii.gov/forms/) and submit it to [email protected] or mail it to the Oahu Office:

For specific records requests related to IWS (e.g., cesspool, septic tanks, aerobic treatment units), please email a completed version of the form provided above to the following email addresses depending on the island your property is on. In the subject line, please include that it is an IWS records request and an identifier for the property in question (e.g., “IWS Records Request for [insert address or TMK here]”. Although we take addresses, TMKs are far more reliable for identifying your property. If you need help identifying your TMK, please reference our TMK guide at: health.hawaii.gov/wastewater/home/tmk-guide/.

What do I do if I did not get a response to my records request, or if my records request was denied?

We typically try to respond to or get in contact with the requester within 10 business days of the request, but weekends, state holidays, and being out-of-office may delay our response a bit. Additionally, extenuating circumstances may have delayed it further as we attempt to determine how to fulfill the request. Please consider contacting us again to check on the status of your request if we have not responded within 10 business days of the request.
