Referral Process

Thank you for partnering with us to connect families to vital services and resources. To make the referral process as simple and efficient as possible, please follow the steps below.

Community Partner Referral

If you are making the referral on behalf of the family, the process is as follows:

    1. Check for Program Availability.

      Before proceeding with the referral, confirm that a home visiting program is available in the family’s community and that the child is in the target age range. Information for each community is below; click on the community name in order to see a detailed map.

    2. Complete the Referral.

      If a home visiting program is available in where the family lives and the child falls within the referral age range, connect families to a Your ʻOhana program. There are two methods to submit a referral:

      Option 1 — Submit an Online Referral Form

      The preferred method is to submit a referral through our secure online portal. This ensures accurate and timely delivery of referral information to the provider in the family’s community and helps them to contact the family with the least amount of delay. Find links to the community-specific Online Referral Form in the table below.

      Option 2 — Contact the Community Referral Coordinator Directly

      If you are unable to use the online portal, please contact the community referral coordinator. They will guide you through the referral process and provide instructions for transmitting the family’s information. Find referral coordinator contract information in the table below.

      If it’s helpful, a paper form can help you gather information from a family that is needed for us to contact them. This form is not required to make a referral.

    3. Next Steps.

      After the referral is submitted, the information is transmitted directly to the local program serving the family’s community, and the program will contact the family to complete the intake process.

Family Self-Referral

Families interested in connecting with Your ʻOhana Program have two simple options:

    1. Complete a referral online.

      Families can visit yourohana.org and complete a referral form themselves. The online form is quick and easy to fill out, and it ensures their information is shared with the right local program.

    2. Contact a program directly.

      Families can also use yourohana.org to find contact information for a local program in their area. They are welcome to call the program to learn more about services and eligibility. As a partner, you may provide the Referral Coordinator phone number from the table above to the family.
