Referral & Eligibility

Kealahou Services Program provides supportive services for youth who are struggling with the trauma they’ve experienced and want to improve their lives. Our referral and eligibility criteria are listed below. Please feel free to contact us with additional questions or concerns.

Referral Process

To refer a youth to the program, please complete the following two forms which can be accessed from the links below.

If you are already receiving services from CAMHD, please inform your care coordinator of your interest with Kealahou Services Program. Referrals can come from many sources. Forms can be filled out by parents, social workers, mental health professionals, school counselor, case managers, or anyone who is assisting the youth being referred. Self-referrals to the program are also accepted

Eligibility Criteria

  • Identifies as a girl
  • Is between the ages of 11-18
  • Resides on Oahu
  • History of traumatic experiences and/or presents with current trauma related symptoms that results in an impairment in daily functioning
  • Has a DSM-5 diagnosis
  • Insurance is not required when applying for services
