The Disease Investigation Section provides the following services:
- For Providers—a list of reportable diseases can be found here. To report a notifiable disease during business hours, please call (808) 241-3565 or send by fax (808) 241-3480. To report a notifiable disease after business hours, please call the Disease Investigation Branch after-hours reporting line, toll-free, at 1-(800)-360-2575.
For the public—to report suspected foodborne illnesses, please call (808) 241-3563.
