If you were vaccinated by the Kauai District Health Office at the Convention Hall clinic, you can log into VAMS and print your vaccine certificate at any time using your email address and the password you created. If you require a new vaccine card, please email the following information to [email protected] and we will call you within 5 business days to notify you that your replacement card is ready for pickup (3040 Umi Street, Lihue).

• Contact # to notify you that your card is ready for pickup

If you weren’t vaccinated by the Kauai District Health Office, please contact the healthcare facility that administered your vaccine to request a replacement.
