Specialty Support Program

The Specialty Support Program (SSP) provides care coordination and other services for children with special health care needs and their families.

SSP services may include information, referral, or support for:

  • Care coordination
  • Nutrition consultation
  • Hearing and vision special services
  • Transition to Adult Health Care
  • Neighbor Island Specialty Clinics (vary by island and may include but not limited to Cardiology, Neurology, and Developmental Pediatrics)

Who is eligible for services?

Children and youth must:

  • Be under age 21 years
  • Reside in Hawaiʻi
  • Have a health condition that will or may last one year or more
  • Be under the care of a specialist
