What is the Case Management Information Services Program?

Community Services for the Developmentally Disabled is responsible for determining eligibility for developmentally disabilities (DD) or intellectual disability (ID) services according to Hawaii Revised Statutes 333F.  If an individual is eligible for services, case managers assist individuals, guardians, and families to create change through a person-centered process based on the individual’s desires, dreams and goals.  Case managers connect individuals to available resources such as Medicaid waiver services, respite funding and 100% state funded services for individuals not eligible for Medicaid.  Community education on developmental and intellectual disabilities, information and referral services are also provided.  Case management offices are located on Maui covering Maui, Lanai and Molokai.
