Obtaining Special License Plates

Obtain Special License Plates (Available only for applicants whose qualifying condition will last six (6) or more years)

To apply for a set of Special License Plates, follow the steps below. (Available only for applicants whose qualifying condition will last six (6) or more years)

  • Complete and sign side 1 of the Parking Permit Application Form (Form PA-1). Mark “I am requesting special license plates” under item 11
  • Have your Hawaii licensed physician/APRN complete side 2 of the form. This step is not required if you have a valid Long Term Disability Parking Placard (Blue)
  • Take the completed form to an issuing agency. Ask the agency for an Application for Disabled License Plates (see below), complete it, and turn both forms in. Show a valid ID and pay the required fee ($6.25 for Maui County and $5.50 for other counties). The issuing agency will issue your special license plates and an accompanying ID card over the counter, except that the City and County of Honolulu will mail your plates to you

Important to note:

  • The applicant must be a registered owner of the vehicle on which the plates will be affixed, and the vehicle must be used primarily to transport the applicant
  • The Special License Plates and the Long Term Disability Parking Placard (Blue) have the same expiration date. When you renew your Long Term Disability Parking Placard (Blue), your Special License Plates will renew as well
  • You must pay your vehicle registration fee to the county where it is registered
