Case Management (Services for the Developmentally Disabled)
Community Services for the Developmentally Disabled is responsible for determining eligibility for developmental disabilities (DD) or mental retardation (MR) services according to Hawaii Revised Statutes 333F. If an individual is eligible for services, case managers assist individuals, families and friends to create change through a person-centered process based on the individual’s desires, dreams and goals. Case managers connect individuals to available resources such as Medicaid waiver services, respite funding and 100% state funded services for individuals not eligible for Medicaid. Community education on mental retardation and developmental disabilities, and information and referral services are also provided. Case managers are assigned geographically to three offices located around the island.
