Chronic Disease Management and Control
Public Health Educators in the County of Hawaii:
- Provide health related information to organizations and businesses with the intent of improving the health of the community and staff.
- Through SNAP-Ed work to increase the knowledge of how low income individuals can use EBT cards at farmers markets to increase consumption of fresh fruits and vegetables in local dishes
- Participate in the development of policy, systems and environmental changes to address social conditions that may reduce chronic disease
